• When will my benefits be effective?

  • • All employees after Open Enrollment:  10/01/2024
    • New Hires:  1st of the month following 30 days from your date of hire

• How do I file an Aflac claim?

  • • Online claim form:  CLICK HERE to go to the Aflac Group Insurance site where you can download a claim form or file your claim online.
    • Paper claim form:  Download the appropriate claim form found on this site and submit your completed claim form to Aflac.  Instructions are on the claim forms.
